Before filing, review the following instructions: 

 

FILLABLE BANKRUPTCY FORMS FOR INDIVIDUALS:

 

Please note: When you file a bankruptcy petition under Chapter 7 or 13, a filing fee is required unless an Application to Pay the Filing Fee in Installments or, in a Chapter 7 case, an Application to Have the Chapter 7 Filing Fee Waived, is simultaneously filed with the Petition.

The current Petition Filing Fees can be found under Court Info: Fees.

The Petition Filing Fee can be paid using one of the following methods:

  1. Online Payment via Pay.gov (See instructions: Pay.gov Instructions)
  2. Debit Card
    • Accepted in person at the Clerk’s Office
    • Credit cards are not accepted from self-represented individuals for payment of the petition filing fees.
  3. Money Order
    • Payable to: Clerk, U.S. Bankruptcy Court or Clerk, United States Court
    • Must be issued by a recognized provider (e.g., post office, bank)
  4. Bank Check (Cashier’s Check)
    • Payable to: Clerk, U.S. Bankruptcy Court or Clerk, United States Court
  5. Cash (In Person Only)
    • Must be in the exact amount.
    • The Court does not make change.
